Rashmi Saluja - Buttock folliculitis wound pus dressing and IM Injection

 

Rashmi Saluja is a graceful, gorgeous, fair 48-year-old woman. She manages a successful import export, with her husband, and is well connected in the social circle. She is very smart and quite active socially. She prefers to dress up in sarees. She is in love with herself and takes good care of her body. She is a very desirable woman for her age. She is a very voluptuous woman. She is fluent in English and French languages.

She walked into my clinic with a visible discomfort on her face. She was wearing a plain black coloured saree, she was looking beautiful, as she is fair and curvy.

Rashmi: Hello Lavanya

Me: Hello Rashmi, what brings you here (I could sense she was uncomfortable)

Rashmi: Kya batau yaar, halat kharab hai 2 dino se.

Me: Kyon, kya hua

Rashmi put a hand on her left buttock, after sitting on the examination table

Rashmi: Yaar yeh buttock par infection ho gaya hai, aur bahut pain karta hai baitho ya chalo toh!

Me: How did that start?

Rashmi, told that she went for a full body waxing at a new parlour and had waxing done on her buttocks as well. She said she wanted to be prepared as her husband was taking her on a Europe trip, her tone got a bit naughty, and her face gave out a slight naughty grin.

Rashmi: Tujhe toh pata hi hoga, waha kya kya hoga. (She winked her eye !) And I had not been to the parlour for a long time, so wanted to be clean. She smiled again!

Me: Okay, I see, can you please lie down on the table, undress and fully expose your buttock, I will have a look.

Rashmi complied as said. She was in pain even when lying on the table, as her buttock was pressed against the table when she turned. She again put her hand on the buttock with a sigh 'ah'! She then lied sideways loosened her saree with one hand and opened her petticoat, then lied on the stomach and slid both saree and petticoat with her black panties to expose her buttocks fully.

Her left buttock showed a large pus filled infection would, it was reddish on the circumference. I palpated it, and it was hot and very tight. Rashmi let out a sigh when I palpated the wound. 

Rashmi: Touch karne se bhi bahut dard karta hai ! Ahh!

Her face was showing the discomfort she was in.

Me: This is folliculitis and its very much aggravated. Pehle aa jaati toh bus topical cream se hi theek ho jata. This is due to waxing, acche se nahi ki waxing parlour main.

Rashmi: Yes, I know, it started itching and becoming painful in the last 2 days, earlier it was not that bad.

Me: Okay, I will have to drain it, clean, and dress it up

Rashmi: Yes but be gentle, as it is hurting a lot

Me: Dear, its aggravated, you will have to bear the pain

Rashmi was not convinced !

I put a white cloth wrap around her buttock, so as to drain the wound.

I used a sterile needle to puncture the wound.

Then I used both by hands and started pressing the wound between both the fingers to remove the pus from it.

Rashmi, was cringing in pain. She let a sigh Ahhhh! Dheere, Dheere !

Me: Thoda ruko, ho raha hai

The wound started draining pus, which I cleaned with the swabs. Then a small amount of blood also came out of the would, after all the pus was drained.

I soaked the swab in disinfectant and then touched the wound with swab

Rashmi gave a swift jerk to her buttock and shouted "Ahhhhh ! it hurts!"

Me: Bas ho gaya, ho gaya.

I cleaned the abscess perfectly removing all pus, blood and debris. Rashmi was writhing in pain during the procedure. I completed the dressing. Rashmi let out a sigh of relief.

Me: Okay, madam, ho gaya aapka kaam. Ab thoda pain hoga chalne, baithne main, but it will be fine at night

Rashmi: Accha pehle hi aa jati aapke pass toh itna dard nahi sahna padtha

Me: Better be late than never dear ! Okay time for an injection

Rashmi: Kya, sui bhi leni padhegi ab

Me: Yes, to stop the infection from spreading.

I prepped the injection and cleaned the spot on her same buttock with cotton swab.

Rashmi: Go slow, you have already given me a lot of pain today

Me: Dont worry, it will be done in a Jiffy!

I darted the needle on her buttock, and injected the medicine fast in one go.

Rashmi: Ouch ! Ahhhh ! Ouiii ! Ouii !

Me: Ho gaya bas !

I pulled the injection, cleaned the spot, removed the white cloth dressing and asked her to dress up

She very slowly pulled her panty up, then pulled her petticoat and then the saree. She lay there putting her hand on her left buttock, while massaging it.

Rashmi: Yaar bahut halat kharab thi, thanks for helping me

Me: Anytime dear !

Rashmi go up slowly with visible discomfort on her face, got down the table, and left with a slow pace, her hand was all the time on her left buttock.
